Terms & conditionsThe Indexmedica UK website is an internet service published by Indexmedica UK using the site the user automatically accepts our Terms and Conditions. We may change and republish the Terms and Conditions from time to time. It is up to the user to regularly review these Terms and Conditions and note any changes. Continued use of the site after a change has been made indicates the user’s acceptance of those changes.
Consultations
Initial consultations are free in Krakow but will be charged for in Wakefield. If you start treatment within 4 weeks of drawing the treatment plan, Indexmedica UK will refund you the cost of the consultation once the treatment is done. The price of the Initial Assessment depends on actual offers and you will be quoted the exact amount prior to finalizing your appointment. During your consultation in Wakefield a panoramic x-ray (OPG) will be required to assess the state of your teeth and mouth. You are responsible for bringing your panoramic X-Ray with you (not older than 6 months). If you don't have one, there is an option to obtain it in Wakefield just before your consultation. Additional fee of £30 for that x-ray have to be paid on day in Wakefield Dental Practice.
Because dental situations can change rapidly, quotations are valid for 90 days and only if there has not been a change in the dental situation which would require additional dental work than that quoted for. Within few days after your Initial Assessment you will receive a personalised Treatment Plan from our dentists in Krakow, where you you will find adequate information on the treatment options available to you, together with their consequences, costs and possible risks. Once receiving the treatment plan, on your request you will be able to discuss with our dentist over the phone, the options available in order to select a course of treatment that is medically sound, aesthetically pleasing and financially affordable to you.
Cancellation
Dental practice in Wakefield operate a verifications system by calling all patients 48 hours prior to appointment, to confirm their attendance. Once confirmed, you can not change the time and date of visit. Should you be late for your appointment dentist can refuse to see you that day and can offer you a different date. Should you not turn up for your appointment at all, Wakefield dental practice will ask you for a deposit of £30 at the time of booking your next appointment.
If, for any reason, you do not present yourself on time and in a state that the consultation can be performed, your appointment will be cancelled, and we will not be able to give you a re-compensation. In certain cases we may need to move your appointment. In these case we will notify you as soon as the need for rescheduling arises, and offer you alternative times.
Cancellations for scheduled dental treatment in Krakow are only accepted in writing (e-mails or post sent directly to Indexmedica UK) and must be received no later than 72 hours (3 working days) in advance. In the case of cancellation for a patient’s first visit, there is no cancellation fee. However, due to the restrictive nature of the timing of treatments, a cancellation penalty of £50 will be charged for the cancellation of a subsequent visit.
Guarantee
All patients get a three year guarantee (from the moment of finishing of the treatment plan) for all fillings and prosthetic restorations made in our clinic. We give ten or five years guarantee for implantological treatment (depending on the system which was used: Nobel Biocare® 10 and 3i® 5 years), one year guarantee for periodontal and relaxing splints.
To claim your guarantee you need to visit our dentist once a year whilst your guarantee is valid. We suggest you to come to Wakefield, where the price of £49 including consultation with your dentist will be charged. You can always visit our Krakow Dental Clinic for a FREE check up, but flight tickets are not covered by Index Medica UK.
In case of small adjustments and repairs, a dentist will see you at our Wakefield practice. In the unlikely event that you require more extensive work, Index Medica will carry out the remedial work at the clinic in Krakow.
Guarantee does not cover:

Additional treatment for example: an X-ray of stitches after surgery, or denture nesting.

We do not approve of any guarantee claims with temporary appliance.

There is no guarantee for endodontic (root canal) treatment.
Diminished or invalidated services within the guarantee:

Negligence of cavity hygiene.

Disobedience to doctor’s suggestions (do not wear complete denture during the night).

Wrong usage of denture. Neglecting control visit dates (once a year)

Natural worn out of denture instrument for example the one holding cavity bone. Untreated dysfunction of cavity joint.

Negative effect on chewing system due to the presence of some common illnesses such as diabetes, epilepsy, osteoporosis, state after chemotherapy, cytostatic therapy.
Liability
IndexMedicaUK (17, Thornton Street, Skipton, North Yorkshire, BD23 1ST, UK) will act as the Representative of Indexmedica (Czapinskiego 2, 30-048 Krakow, Poland). It is acknowledged by you that IndexMedicaUK does not provide dental services and it is expressly agreed that no liability whatsoever will attach to it arising out of the performance or non performance of these Trading Terms or the Treatment Plan or any modifications to it.
IndexMedicaUK's staff may assist you with the booking of hotels, apartments, transfers, sightseeing but you will be solely responsible for settling all and any costs arising directly with the service provider/operator. Under no circumstances whatsoever will IndexMedicaUK be liable for any losses howsoever arising from the performance or non performance of any service provider/operator (flight delays, luggage loss. etc.) We strongly recommend to buy Travel Insurance for every single person travelling for dental treatment in Poland.
IndexMedicaUK can help with hotel, appartments, sightseeing arrangements but all customers are responsible for any cancellation. All fees or charges imposed by other travel suppliers, such as airline or hotel will be paid by customer. Please note that for some hotels or apartments once your reservation is confirmed, your reservation may be wholly non-refundable.
In addition to any liability expressly or by implication excluded under these Trading Terms, IndexMedicaUK shall not be liable for consequential losses and its liability shall not in any case exceed a total amount equivalent to the amount payable by the patient for the treatment he/she received
While we have tried to provide accurate and timely information, there may be technical or factual inaccuracies and typographical errors, for which we apologise and we accept no liability for any inaccuracies or omissions on our website to the extent permitted by law. We reserve the right to make changes and corrections at any time, without notice.

Should you have any complaints with the dental work performed, we suggest that you immediately contact IndexMedicaUK Personal Assistant on 0800 3345967or

It is helpful to raise any concerns you have about the service as soon as possible in order to resolve this at an early opportunity and that all complaints are taken seriously. In order to perform our services you will be required to disclose certain personal information, including your personal data, travel plans and health records to IndexMedicaUK or any agent appointed by it to receive such information. No liability will arise in respect of any losses suffered by you where the Information disclosed is found to have been incorrect or incomplete. It is your sole responsibility to ensure that the Information provided is accurate and to immediately notify IndexMedicaUK, or any agent appointed by it to receive such information, of any changes to it.
IndexMedicaUK will always respect your privacy and any personal communication between yourself and itself or any associated company. The information that IndexMedicaUK collects from you is only that required by IndexMedicaUK or any associated company to provide you with the information, products or services requested. IndexMedicaUK does not trade, rent or sell client details to third parties. Should IndexMedicaUK intentions change it will be done only with your authorisation. IndexMedicaUK will always comply with any data protection legislation currently in force.
